The first Ohio Family Health Survey (OFHS), in 1998, was developed by the Ohio Department of Health to obtain baseline statewide data on a number of health-related matters. Telephone interviews of approximately 16,000 Ohio households were conducted with one adult in each household. Information was also gathered on nearly 6,000 children from interviews with adult family members.
The 2004 Ohio Family Health Survey, sponsored by the Ohio Department of Job and Family Services, was a substantial expansion of the original survey, obtaining responses from nearly 40,000 adults and gathering information on more than 15,000 children.

The 2004 and 1998 OFHS public use dataset and documentation are available for download below. This public use dataset contains data collected from the adult and child questionnaires, except for select variables relating to the administration of data collection.
